If you have special forms you would like us to include in your demonstration, please fax them to 678-623-3217. Please include a cover page with your practice name, contact name, specialty, and contact information.

Please fill out the form completely; it will help us provide a better demonstration and communicate with you more efficiently. We send out web demonstration meeting invitations through email. If you choose not to divulge your email, it may be more difficult to join the web meeting.

In addition, please consider filling out our online needs analysis. Our comprehensive needs analysis helps us understand what your practice requires for a successful EHR implementation. We also use the needs analysis to build a quote for your system.
